Radiofrequency Ablation as a Method of Local Tumor Control: Current Role in Oncology
An organ-preserving approach and minimizing invasiveness are among the leading trends in modern oncology, where radiofrequency ablation (RFA) holds a prominent place. For a practicing physician, this is primarily an effective way to radically and safely affect tumor foci in clinical situations where open resection is impossible or too risky for the patient.
At the same time, remember: RFA is not a universal replacement for surgery or chemotherapy. It is an independent, high-precision method for situations requiring effective local tumor control.
RFA: What kind of method is it
RFA is a minimally invasive technique for locally destroying tumor tissue with heat. A high-frequency current (350–500 kHz) is applied to the focus through a puncture needle-electrode under ultrasound or CT guidance. This causes ion agitation in the tissues, heats the area to 60–100 °C, and kills tumor cells through coagulation necrosis.

What place does RFA occupy among local treatment methods
Among local treatment methods, RFA falls between surgical resection and active surveillance. Interventional oncology and local tumor ablation
Modern interventional oncology combines high-precision visualization and minimally invasive access, allowing for targeted tumor destruction without significant incisions. In this context, local tumor ablation becomes an indispensable tool.
What tasks can local tumor ablation solve:
- Radical removal of early tumors in patients with high surgical or anesthetic risk.
- Control of oligometastases (in particular, RFA is effective for liver, lung, kidney, and bone tumors).
- Cytoreduction to decrease tumor burden.
- Palliative relief of pain syndrome (in particular with bone metastases).
RFA as part of multimodal treatment
RFA is rarely used in isolation: it is successfully combined with chemo-, targeted, and immunotherapy, and the sequence of stages must be approved by a multidisciplinary oncology council.
Patient selection for radiofrequency ablation
Careful selection is the main factor in relapse-free survival and patient safety.
What tumor characteristics are considered:
- Number of foci (usually up to 3–5).
- Exact linear dimensions in three projections.
- Anatomical location and distance to vessels/ducts.
- Safety of access for electrode insertion.
Remember that a nodule on a scan is not an automatic indication for RFA. Decisive factors include overall performance status (ECOG), functional organ reserve (e.g., Child-Pugh for the liver), tumor growth rate, and response to prior systemic treatment.

Local tumor control: How the result of RFA is evaluated
Treatment quality is evaluated based on clear radiological criteria of complete destruction, not just on the technical act of performing the procedure.
In 1–3 months, contrast-enhanced CT or MRI is performed (according to mRECIST criteria). A successful ablation looks like a clearly defined avascular zone without contrast accumulation.
Preserved blood flow in the focus indicates residual tumor tissue, and new contrast areas along the periphery indicate relapse, which requires repeat ablation or a change in treatment tactics.
Advantages and limitations of RFA
The method has a clear balance between clinical benefit and anatomical-technical limits.
What are the potential advantages of the method:
- Low trauma and short hospital stay (1–3 days).
- Possibility of execution under local anesthesia or sedation.
- Maximum preservation of functional organ parenchyma.
- Possibility of repeated application in case of new foci.
- High safety standards (additional sensors, water cooling, etc.).
What factors may limit the application:
- Tumor size over 4–5 cm.
- Diffuse organ involvement.
- Uncorrected coagulopathy.
- Risk of thermal damage to adjacent major vessels or intestines.
RFA in a comprehensive strategy for treating an oncological patient
RFA can significantly improve prognosis and quality of life for patients with oligometastatic disease.
